How to Apply for a Driver's Abstract
Option 1: Request Your Abstract In Person (Recommended)This is the simplest and most efficient way to obtain your abstract.
● Fill out an application (Request for Personal Driving and Motor Vehicle Information (PDF, 1 page))● Drop off the application● Bring your completed application to a registry agent.● Provide an accepted form of photo ID.● Sign and date the form in front of the registry clerk.
Option 2: Authorize Someone to Request on Your BehalfIf you're unable to visit a registry in person, someone you trust can request the abstract for you.
● You fill out Driver Abstract Consent (PDF, 1 page, Check box 5(1)(a) and complete Part 1 only)● Your representative fills out the following: Driver Abstract Statement of Intent (PDF, 1 page)● They must bring a valid photo ID.● Submit the documents● Send your forms directly to your representative or the registry agent.
Your representative must apply in person and sign the form in front of the registry clerk, who will also witness the signature.
Option 3: Requesting While Outside AlbertaIf you're currently outside Alberta, follow these steps:
● Fill out an application (Notarized Request for Personal Driving and Motor Vehicle Information (PDF, 1 page))● Include a copy of the accepted photo ID.● Visit a Notary Public● The notary must verify your identity and notarize both the application and ID copy.● Sign the form in front of the notary.● Send the completed application to an Alberta registry● Include the notarized form, ID copy, and any applicable fees.

Option 4: Requesting a Minor's Driver AbstractA parent or legal guardian may apply on behalf of a minor (under 18) who is not married or self-supporting.● Fill out the application: Driver Abstract Statement of Intent (PDF, 1 page (Check box 5(1)(b)(iv))● Submit in person● Bring a valid photo ID and the completed form to the registry.● Sign the form in front of the registry agent clerk, who will witness the signature.
